Description

Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Required Qualifications:

  • Master's Degree - Social Work.
  • Coursework/Training - All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
  • California Clinical Social Worker License upon hire.
  • Designated as Behavioral Health Clinician to perform 5150 evaluations within 18 months of hire.
  • CPI certification (nonviolent crisis intervention training) within 1 year of hire.
  • 2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
  • Computer experience.

Preferred Qualifications:

  • Discharge planning experience.
